Abstract The rapid integration of renewable energy sources has fundamentally changed how modern power grids respond to electrical faults. Because inverter-based resources lack the physical momentum of traditional spinning generators, detecting complex anomalies like high-impedance faults and inverter failures has become increasingly difficult. Although recent research proposes sophisticated, data-driven diagnostic algorithms to address these challenges, a significant disconnect persists between theoretical models and practical utility deployment. This paper bridges that gap by critically reviewing current fault detection and mitigation strategies strictly from an operational perspective. We examine a major shortfall in the literature: computational models trained on synthetic data often fail when exposed to real-world, noisy grid conditions. To address this vulnerability, we analyze documented challenges from electric utilities and evaluate the actual technical readiness of both established and AI-driven diagnostic methods. The review also assesses the deployment barriers facing commercial mitigation technologies, specifically energy storage systems and grid-forming inverters. By examining these industry realities, we identify the necessary steps to transition from academic simulations to genuinely resilient, field-ready fault management frameworks.
