This scoping review, conducted according to PRISMA-ScR guidelines and the JBI framework, systematically maps current technologies, operational strategies, and engineering innovations used to optimize electrical energy consumption and reduce carbon footprints in operating rooms. By evaluating all existing methods, ranging from HVAC optimization to AI-driven management systems, this review aims to synthesize the current surgical sustainability landscape and explicitly identify the methodological gap regarding kinetic energy harvesting. Ultimately, this work establishes the rationale and empirical foundation for introducing a novel conceptual framework: AI-enabled, energy-harvesting smart flooring designed to capture intraoperative mechanical energy and convert it into clean electricity within surgical suites.
