📄 Abstract(原文)

Healthcare travel is a hidden contributor to health inequality and NHS-related carbon emissions. The Community Responsive Digital Twins for Inequality Alleviation and Low-Carbon Mobility (CORDIAL) project explored how lived experience can inform fairer, lower-carbon healthcare transport. Drawing on three in-person Patient and Public Involvement and Engagement groups and four online conversations with 15 contributors in Southampton and the Isle of Wight, this report identifies key barriers to accessing care.<br><br>Contributors described costly, unreliable and indirect public transport; early appointment times; poor wayfinding; limited rescheduling options; stressful return journeys after treatment; parking pressures; and reliance on fragile voluntary transport services. These barriers were felt most strongly by people living with long-term conditions, limited income, mobility needs, rural or island geography, and limited support networks.<br><br>The findings show that decarbonising NHS travel must be designed around patients’ lived realities, not simply through encouraging public transport use. CORDIAL provides a foundation for future digital twin tools that integrate lived experience with NHS, transport and spatial data to identify priority areas for fairer, more accessible and lower-carbon healthcare mobility.
