Climate change is one of the most serious threats to life, with hospitals contributing substantially to greenhouse gas (GHG) emissions due to their intensive energy consumption, resource use, and waste generation. This study assessed the contributions of selected Level 3 hospitals in Metro Manila to reducing GHG emissions in terms of Scope 1 (direct emissions), Scope 2 (indirect emissions from purchased energy), and Scope 3 (other indirect emissions from the supply chain). It also examined whether contributions differed significantly when profile variables were considered, identified environmental actions promoting sustainability across ten interconnected goals—leadership, chemicals, waste, energy, water, transportation, food, pharmaceuticals, buildings, and purchasing—and determined the relationship between GHG reduction contributions and environmental actions. Using a descriptive–correlational design, data were collected from 67 purposively selected respondents representing ten public and private Level 3 hospitals through a validated researcher-made questionnaire (Cronbach's α = 0.949). Data were analyzed using weighted mean, ANOVA, t-test, and Pearson correlation. Results showed that hospitals "moderately contributed" to reducing GHG emissions, with Scope 2 ranking highest and Scope 3 lowest. Significant differences were found for Scopes 2 and 3 by hospital classification, and for Scopes 1 and 3 by length of service, while no significant differences were observed for sex. Waste management was the most observed environmental action, while food-related sustainability ranked lowest. A low but significant positive correlation existed between GHG reduction contributions and environmental actions. Based on these findings, a Unified Environmental Sustainability Framework was developed to enhance compliance, operational efficiency, sustainability, and financial performance.
