Whether China’s emission trading system (ETS) promotes regional “inward efficiency gains” or induces cross-regional “carbon leakage” — a question that bears directly on the country’s ability to meet its dual carbon targets of peaking emissions by 2030 and reaching neutrality by 2060. Drawing on a panel of 30 Chinese provinces spanning 2007–2024, this study measures carbon emission efficiency (CEE) with an undesirable-output slacks-based DEA model, then applies the group-time estimator of Callaway and Sant'Anna (2021) to the staggered rollout of China’s ETS pilots, treating the policy’s phased introduction as a quasi-natural experiment. The estimates point to a significant average treatment effect of the ETS on CEE (ATT = 0.065, p < 0.05), with policy effects exhibiting pronounced dynamic accumulation over time. Mechanism analysis indicates that this efficiency improvement operates primarily through reductions in energy consumption intensity (ATT = −0.117, p < 0.01), consistent with the “inward efficiency gains” hypothesis. Spatial spillover analysis reveals moderate carbon leakage during the early pilot phase (Border_ETS coefficient = 1.006, p < 0.05),an effect that faded once the national unified carbon market began operating in 2021. By integrating regional regulatory heterogeneity, energy efficiency adjustment, and the transition toward a unified national carbon market into a coherent analytical framework, this study identifies a dynamic pathway of inward efficiency gains, early-stage carbon leakage, and subsequent coordinated emission reduction that traces the evolution from localized pilots to a unified national market, providing empirical evidence to inform national carbon market development and regionally coordinated emission reduction strategies.
